Hyperosmolar non-ketotic diabetic coma and rhabdomyolysis.

Rhabdomyolysis is a rare but potentially fatal complication of hyperosmolar states. We report a case of severe hyperosmolar non-ketotic diabetic coma causing rhabdomyolysis in a young man. Despite very high levels of creatine kinase there was no detectable myoglobinuria. Creatine kinase estimation should be a standard investigation in all patients presenting with a hyperosmolar state.

Hyperosmolar hyperglycaemic non-ketotic diabetic coma complicating open heart surgery.

The syndrome of hyperosmolar hyperglycaemic non-ketotic diabetic coma was first described by Sament and Schwartz in 1957. In a recent publication, McCurdy (I970) reviewed the details of 84 cases. Various possible aetiological factors have been considered, and in an Editorial in journal of the American Medical Association (I968), attention was drawn to an iatrogenic element. It has complicated p...

Hyperosmolar non-ketotic diabetic coma induced by furosemide in modest dosage.

Introduction Thiazides are known to diminish glucose tolerance and precipitate or aggravate diabetes, but that furosemide may do so is not so well appreciated (Walsh and O'Sullivan, 1974; Solvsteen, Olsen and Hansen, 1968; Toivonen and Mustala, 1966; Heimsoth and Bock, 1970). Large doses of furosemide may be a factor responsible in causing hyperosmolar non-ketotic diabetic coma (Tasker and Mitc...

Hyperosmolar non-ketotic diabetes.

Hyperosmolar non-ketotic crises occur chiefly in mild, maturity-onset diabetics, yet present a medical emergency carrying a high mortality. This may in part be due to a lack of awareness of the clinical presentation, and of the principles of treatment. Five cases are described, in all of which excessive consumption of Lucozade, a proprietary 'glucose' drink, played an important part in the deve...

Hyperosmolar non-ketotic diabetic coma: with particular reference to vascular complications.

Two West Indian men with no previous history of diabetes mellitus developed hyperosmolar non-ketotic diabetic coma. Intra-abdominal catastrophes secondary to mesenteric thrombosis played a major part in the death of these patients, in both of whom control of the hyperosmolar state had been achieved. Both patients had evidence of infarction of intestine at necropsy. Vascular thromboses are a maj...
